At Care New England, we’re seeking dedicated OTs to join our Home Health team and deliver personalized, goal-driven care that empowers patients to live as independently and safely as possible.
What You’ll Do
- Manage a caseload of patients from admission through discharge, designing and implementing treatment plans that address individual needs and goals.
- Collaborate with an interdisciplinary team and support patients through education, adaptive strategies, and therapeutic interventions—all from the comfort of their own home.
- Conduct comprehensive assessments and functional evaluations.
- Develop personalized treatment plans focused on independence and safety.
- Implement therapy sessions to improve mobility, strength, and daily living skills.
- Educate patients and caregivers on exercises, equipment use, and home safety.
- Adjust care plans based on patient progress and clinical findings.
- Collaborate with nurses, physical therapists, speech therapists, and case managers.
- Document care in accordance with agency policies and regulatory standards.
- Participate in team meetings and quality improvement initiatives.

What You’ll Need
- Education and credentials required for RI Occupational Therapy licensure (required).
- Bachelor’s Degree in Occupational Therapy or related discipline (strongly preferred).
- Valid Rhode Island OT license and CPR certification (required).
- At least one year of progressive clinical experience (home health or acute care preferred).
- Valid driver’s license and automobile liability insurance.
- Excellent clinical, documentation, and communication skills.
