Responsibilities
- As a key member of our care team, you’ll work under the supervision of a Case Manager to deliver direct personal care and support to patients at home.
- You’ll help patients with daily living activities, provide companionship, and monitor their condition—while keeping the care team informed every step of the way.
Your Day-to-Day Might Include
- Assisting patients with bathing, dressing, grooming, and personal hygiene
- Helping with toileting needs and mobility support
- Performing light housekeeping duties, changing linens, and preparing meals
- Reporting food and fluid intake/output as assigned
- Taking and recording vital signs under professional supervision
- Providing medication reminders and emotional support
- Observing and reporting any changes in patient condition or behavior
- Documenting care accurately and communicating with the care team
- Supporting therapy exercises as directed
- Calling to confirm scheduled visits and respecting each patient’s routine

What You’ll Need
- High school diploma or equivalent
- Current Rhode Island CNA certification (required)
- CPR certification (required)
- Valid driver’s license and automobile liability insurance
