At Care New England, we are seeking dedicated OTs to join our Home Health team and deliver personalized, goal-driven care that empowers patients to live as independently and safely as possible.

What You’ll Do

  • Manage a caseload of patients from admission through discharge, designing and implementing treatment plans that address individual needs and goals.
  • Collaborate with an interdisciplinary team and support patients through education, adaptive strategies, and therapeutic interventions—all from the comfort of their own home.
  • Conduct comprehensive assessments and functional evaluations.
  • Develop personalized treatment plans focused on independence and safety.
  • Implement therapy sessions to improve mobility, strength, and daily living skills.
  • Educate patients and caregivers on exercises, equipment use, and home safety.
  • Adjust care plans based on patient progress and clinical findings.
  • Document care in accordance with agency policies and regulatory standards.
  • Participate in team meetings and quality improvement initiatives.

What You’ll Need

  • Education and credentials required for RI Occupational Therapy licensure (required).
  • Bachelor's Degree in Occupational Therapy or related discipline (strongly preferred).
  • Valid Rhode Island OT license and CPR certification (required).
  • At least one year of progressive clinical experience (home health or acute care preferred).
  • Valid driver’s license and automobile liability insurance.
  • Excellent clinical, documentation, and communication skills.
