The predicted salary is between 28800 - 43200 £ per year.
Under the supervision of the Manager, is responsible for providing social work consultation to patients, staff members, and community representatives. Provides quality medical-social work care for assigned patients with social related problems. These responsibilities will be carried out in accordance with organization policies/procedures and standards consistent with Federal, State and accreditation regulations, as well as Organization and other regulatory organizational requirements. Serves as a liaison between agency and other community organizations.
Duties and Responsibilities:
- Assists staff in setting boundaries with patient/caregiver. Develops and implements agreements with family.
- Identifies potentially high-risk patient/client conditions/situations; evaluates and determines the degree of risk; responds in a manner dictated by nature and urgency of situation.
- Refers all unsafe situations to appropriate state agencies.
- Consults with and provides support services to family, patients/clients, and other community resources; assesses family financial situation considering patient/client’s prognosis and health care needs; refers family to community services or other agencies for financial assistance, as appropriate and needed. Hospice SW facilitates respite stays and assists with bereavement/pre-bereavement needs.
- Researches other potential sources of financial assistance (Medicaid, SSI, etc.) if warranted to connect pt/client to needed services.
- Participates in and leads case conferences.
- Serves as a resource/consultant for hospice/home health.
- Participates in the training and in-service education of Social Services Staff and other health professionals providing consultation, peer-to-peer supervision/evaluation and teaching as assigned. LICSW may provide supervision for BSW, LCSW as applicable.
- Maintains continuous communication with all personnel related to assigned case load regarding patient/client/family strengths and areas for improvement, special needs of patient/client/family, changes in physician orders, and, as required, requests for additional health care services.
- Prepares, maintains and submits all required paperwork, comments, records and forms related to patient/client care in an accurate, complete and timely fashion and in accordance with organization policy/procedures and all regulatory requirements.
- Other duties as assigned.
Requirements:
- Bachelor’s Degree in Social Work required (BSW).
- Master’s Degree in Social Work preferred.
- MSW: LCSW/LICSW preferred.
- Continuing education courses in area(s) of interest preferred. Knowledge of computer systems preferred.
- Minimum of 3-5 years social work experience required, within a home health care setting preferred.
- Experience working with medical social services or community resources for Hospice or Therapeutic patients.
- Must possess a valid driver’s license and current auto liability insurance.
