At VNA of Care New England, we are seeking dedicated RNs to join our Hospice team and provide individualized, one-on-one care that empowers patients in the comfort of their own homes.
What Youll Do:
As a Hospice RN, youll manage a caseload of patients working as part of a dedicated interdisciplinary team. Youll assess, plan, and coordinate care while monitoring and adjusting treatment as needed to promote the best possible outcomes.
Your Day-to-Day Might Include:
Performing comprehensive assessments and developing personalized care plans
Coordinating care and referrals across disciplines and specialties
Monitoring patient progress and adjusting care to achieve optimal outcomes
Communicating with patients, families, and providers to ensure continuity of care
Educating patients and caregivers on managing conditions at home
Documenting clinical notes and updates with accuracy and clarity
Identifying and addressing opportunities for care improvement

What Youll Need:
RI Registered Nurse license (required)
Bachelors Degree in Nursing or related field (strongly preferred)
At least 1 year of clinical nursing experience (hospice preferred)
Current CPR certification
Valid drivers license and automobile liability insurance
Excellent written and verbal communication skills
Strong clinical judgment and problem-solving abilities
