- Board certified Nurse Practitioner or Physician Assistant, palliative care experience is a plus, new grads considered if certified
- Join an experienced team of providers with residents and fellows on site
- Patient population: 18 years old+ in need of palliative care, average in-patient census: 5
Responsibilities include:
- provide guidance and support to entire inpatient hospital population for management of symptoms related to chronic or advanced disease, including pain
- be comfortable with complex goals of care conversations
- possess the ability to work as part of an interdisciplinary team

- possess baseline knowledge of post- acute care services
- be comfortable practicing with autonomy and at the top of their license
- ability to speak Spanish and/or Portuguese is a plus
