Job Description

Under the supervision of the Team/Program Manager, the HH clinician is responsible for managing or caring for a caseload of patients from admission to transition into the community. This involves interdisciplinary support and referrals when appropriate. The position coordinates interdisciplinary care, monitors and adjusts treatment to achieve positive therapeutic outcomes, and identifies opportunities for process improvement. All responsibilities are carried out in accordance with organizational policies, procedures, and standards consistent with Federal, State, and accreditation regulations.

Requirements

  1. Equivalent education required for RI LPN licensure; Bachelor’s Degree (minimum) in a relevant discipline is strongly preferred.
  2. Valid RI LPN license required.
  3. Minimum of one (1) year of progressive clinical experience required; experience in an acute care or home health care setting is preferred.
  4. Valid CPR certification required.
  5. Valid driver’s license and automobile liability insurance are required.
  6. Ability to read and interpret documents such as medical records and reports. Ability to write reports, business correspondence, and procedure manuals. Excellent oral and written communication skills in English are required.
  7. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret technical instructions in written, oral, diagram, or schedule form. Must handle several variables, both abstract and concrete.
