Care Lync is seeking a Support Coordinator/Service Coordinator to manage caseloads for adults with intellectual, developmental, and physical disabilities. You will develop individualized care plans, coordinate services, and monitor outcomes to ensure cost-effective, quality care.
Ideal candidates hold a BS/BA, are proficient with Microsoft Office and state databases, and maintain CPR/First Aid certifications. A valid driver’s license is required and bilingual English/Spanish is a plus.
