Care In Hand is seeking an experienced and motivated Care Coordinator to join our established domiciliary care team.
With over 20 years’ experience in elderly domiciliary care across Pembrokeshire and West Carmarthenshire, Care In Hand is recruiting an ambitious, hardworking team player, with relevant experience and qualifications within the domiciliary home care industry. The Care Coordinator role will be 40 hours per week over 4 days including some weekends, shift times are 0700 – 1700 or 1200 – 2200. There is no on call duty. We have a team of call monitors.
Duties
- You will be 1 of 4 Care Coordinators and your main duties will include:
- Manage a specified area in Pembrokeshire of 600 hours per week of client’s hours and on average 30 carers.
- Prepare fortnightly staff rotas.
- Schedule standby carers to cover absences.
- Ensure person centred care plans / assessments outcome are established, renewed, maintained and implemented for all clients.
- Complete various staff reviews, supervisions, and annual appraisals.
- Ensure that Field Supervisors carry out all their duties and responsibilities.
- Work with Registered Manager, Field Supervisors, Call Monitors, Operations team, Commissioning, Human Resource, Training, Finance, PR / Tech Support and Wellbeing Coach to ensure regulations are met.
- Ensure the agency provides and maintains the highest levels of care and service in accordance with the company’s mission, vision, policies and procedures.
- To manage effectively and efficiently all allocated resources.
- To work with other professional colleagues to ensure the clients best interests are identified and plans established to meet them.
This role will be offered with the assistance of extensive training for the first 3 months of probation with the allocation of the Registered Manager to induct you to the new role.
