Job Description
Position: Support Worker / Care Worker
Location: London/Essex/Basildon
Hours: Full-time / Part-time / Shift pattern
Reports To: Senior Care Coordinator
Role Purpose
To provide high-quality, person-centred care to an individual with complex needs, including learning disabilities, autism, challenging behaviours, sensory disorders, and medical needs requiring PEG feeding and medication administration. The role focuses on enhancing quality of life, promoting independence, ensuring safety, and supporting the patient’s physical, emotional, and social needs.
Key Responsibilities1. Personal Care & Support
- Assist with daily living activities including bathing, dressing, toileting, and grooming.
- Support the patient to achieve personal goals and maintain independence.
- Encourage participation in social, recreational, and educational activities.
2. Complex Needs Care
- Understand and respond appropriately to challenging behaviours, using positive behaviour support strategies.
- Support sensory needs, including managing hypersensitivity or sensory processing difficulties.
- Implement individual care plans tailored to the patient’s autism, learning disability, and sensory requirements.
3. Medical & Clinical Support
- Administer medication as prescribed, including via PEG (Percutaneous Endoscopic Gastrostomy) following safe administration protocols.
- Monitor and report any adverse reactions, changes in condition, or health concerns to the nursing or medical team.
- Assist with PEG feeding, ensuring hygiene, safety, and accurate record-keeping.
4. Observation & Documentation
- Maintain accurate records of care, medication administration, behaviours, and incidents.
- Contribute to care planning meetings and reviews with the multidisciplinary team (MDT).
- Report any safeguarding concerns promptly according to organisational policy.
5. Communication
- Communicate effectively with the patient, family, and multidisciplinary team.
- Use appropriate communication methods, including visual aids, Makaton, or other communication tools suitable for autism or learning disabilities.
6. Health & Safety
- Follow infection control, health and safety, and safeguarding procedures at all times.
- Ensure the patient’s environment is safe, comfortable, and free from hazards.
- Respond calmly and safely to challenging behaviours or emergencies.

Essential Skills / Attributes:
- Experience supporting individuals with learning disabilities, autism, or challenging behaviour.
- Knowledge of PEG feeding, medication administration, and basic medical procedures.
- Understanding of sensory processing disorders and ability to support sensory needs.
- Strong interpersonal and communication skills.
- Patience, empathy, and resilience.
- Ability to work as part of a multidisciplinary team.
- Knowledge of safeguarding, health, and safety policies.
