To provide high-quality, person-centred support to individuals with learning disabilities and/or mental health needs, enabling them to live as independently as possible. The role focuses on building skills, confidence, and resilience while promoting dignity, choice, and control over their own lives.
Key Responsibilities
- Support individuals to develop independent living skills (e.g. cooking, budgeting, personal care, travel training)
- Promote choice, control, and decision‑making in all aspects of daily life
- Provide emotional support and encouragement to maintain positive mental health and wellbeing
- Support individuals to access the community, education, employment, and social opportunities
- Assist with personal care where required, in a respectful and dignified manner
- Support with medication in line with training and policies
- Follow and implement person-centred support plans and risk assessments
- Use positive behaviour support (PBS) approaches where needed
- Build trusting, professional relationships while maintaining appropriate boundaries
- Monitor and report changes in behaviour, mood, or wellbeing
- Maintain accurate, timely, and clear records
- Work collaboratively with families, healthcare professionals, and other agencies
- Safeguard individuals and report any concerns in line with procedures
