Your Role As a key member of our interdisciplinary care team, you will work under the supervision of a Clinical Manager to coordinate and deliver high-quality, patient-centered care. You’ll play a vital role in ensuring comfort, dignity, and quality of life for those we serve.
Key Responsibilities
- Perform ongoing, comprehensive assessments of patients and families
- Develop and oversee individualized care plans in collaboration with the care team
- Lead and participate in interdisciplinary case conferences
- Coordinate care with physicians, hospice staff, volunteers, and community partners
- Advocate for patients and families, addressing clinical, emotional, and psychosocial needs
What You’ll Bring
- Active Massachusetts RN license in good standing
- Associate’s degree in Nursing (BSN preferred)
- Experience providing compassionate care in home or facility-based settings
- Strong interpersonal and communication skills
- Ability to work independently while thriving in a team environment
- Sound clinical judgment, flexibility, and strong organizational skills
- Valid driver’s license and access to a reliable, insured vehicle
