What You’ll Do

  • Craft personalized care plans in collaboration with residents, families and healthcare professionals to design care that is as unique as the individual.
  • Ensure accuracy of care documentation; maintain legal compliance and clarity for the team.
  • Lead each shift by organizing and guiding the team, ensuring everyone understands their roles.
  • Facilitate seamless transitions through effective shift handovers and continuity of care.
  • Monitor health through routine observations and attend to specific health needs with attention to detail and safety.
  • Administer medications safely, adhering to MAR sheets and protocols.
  • Build strong relationships with residents, families and healthcare professionals; address concerns with empathy and urgency.
  • Promote independence by supporting daily activities and providing hands-on assistance as needed.
  • Act on feedback to prioritise resident health and wellbeing.
  • Champion respect by upholding dignity and individuality of every resident.
  • Encourage engagement by facilitating social interactions and activities within a vibrant community.
  • Prioritise safety and maintain a secure environment for all.
  • Support nutrition by monitoring hydration and weight changes for residents at risk.
  • Respond to emergencies promptly, using the on-call system and reflective practices to improve care.

About You

  • You are a Registered General or Mental Health Nurse (RGN or RMN) with an active NMC Nursing PIN.
  • Care home experience is a plus, but passion for care and dedication to residents’ lives matter most.
  • You naturally connect with residents, families and colleagues with warmth, understanding and a calm, patient demeanour.
  • Excellent communication skills; you convey needs, updates and guidance with clarity, empathy and respect.
