The predicted salary is between 26000 - 30000 £ per year.

An exciting opportunity has arisen for an ALN Transition Worker (Placements), working as part of the wider Inclusion Team within the Education Service and with partners in many different agencies. The role focuses on working with parents/carers ensuring that the transition into specialist provision is as smooth as possible for children with additional needs and disabilities. The successful candidate will focus on placements through from early years up to Post 16, as well as working as a lead contact with our Welsh language provisions.

We are looking for a highly organised, motivated and passionate individual who can join our dynamic forward-thinking team to improve outcomes for children through partnership working. The successful candidate must have excellent communication skills both verbal and written. They will need to be able to manage difficult conversations as part of disagreement resolution., This post is subject to an Enhanced Disclosure and Barring Service check.

About The Service

The Inclusion Service work with, support and coordinate services for children and young people with additional needs and disabilities, their families and settings/schools to ensure that children and young people access their entitlement with smooth transitions into a range of settings. We work in partnership with parents, education services, and other professionals in education and health.

The Inclusion Team monitors and supports schools/settings to successfully include all children and young people with additional needs and disabilities to access their entitlement, environment and make progress.
