- Motivated to contribute to service development, innovation and quality improvement
Key Responsibilities:
- Conduct face-to-face and telephone consultations, including home visits
- Deliver weekly planned care home ward rounds in line with the Enhanced Health in Care Homes (EHCH) model, ensuring proactive, personalised care for residents

- Build continuity relationships with patients, carers, and care home staff
- Collaborate with GPs, Community Rapid Response services, Falls teams, and other MDT partners
- Contribute to the ongoing development of the Home Visiting Service
- Participate in daily debriefs, monthly team meetings, and regular supervision
