Experience Practice Nurse Aldermoor Surgery, Southampton
The Practice Nurse will be responsible for the delivery of high quality nursing services as part of the practice multidisciplinary team The role will include providing routine nursing care chronic disease management health promotion health screening vaccinations wound care travel medicine and supporting patients with long term conditions The Practice Nurse will assess patients develop and review treatment plans provide appropriate clinical interventions and refer patients to other services where required The role will involve maintaining accurate clinical records supporting safeguarding and clinical governance and working within professional and practice guidelines The Practice Nurse will contribute to infection prevention and control quality improvement clinical audits and the development of practice nursing services The successful candidate will work collaboratively with GPs and the wider clinical team to provide safe effective and patient centred care The Practice Nurse will support patients with health promotion prevention and management of their health and wellbeing and will undertake any other duties appropriate to the role and level of competence
The ideal candidate will have experience within Primary Care, be adaptable, highly motivate and enjoys working as part of a team. The post holder will assist in achieving QOF targets and ensure that the associated admin and clinical duties are complete. We would require the post holder to have experience in chronic disease management (Asthma, COPD, and Diabetes) The post holder would also be required to manage our infection control.

Our practice is based on the west side of Southampton. We are part of the West Primary Care Network, and we serve just over 8,455 patients. We have a varied and diverse population consisting of working families/professionals and the retired elderly. We are also a training practice and support 3 GP registrars and medical Students. Aldermoor surgery is a friendly and supportive practice and has a strong commitment to our patients and staff and their professional development. We promote good work life balance.
Our team comprises of 4 partners, 3 salaried GPs, 1 practice nurses, and 2 HCAs, Pharmacist and Mental Health Nurse. Aldermoor is also a training practice.
