Social Prescriber

This role combines social prescribing, health promotion, and smoking cessation support specifically for Gypsy, Roma and Traveller communities across Surrey. The post holder will: Build trusting relationships with GRT individuals and families Provide personalised support to improve wellbeing using Make every Contact Count (MECC) Deliver smoking cessation advice, behavioural support and followup Support access to primary care, screening, vaccinations and wider services by signposting and referring into appropriate services Work collaboratively with ABCs Health Inclusion Teams, VCSE partners and local authorities This role is ideal for someone passionate about reducing health inequalities, working in community settings, and supporting people in a culturally sensitive, trauma informed way. Genuine Occupational Requirement Due to the specific safeguarding needs and privacy considerations of the service users in this role, including required access to homes where individuals are of a particular sex and where sensitive, gender specific engagement is essential, it is a proportionate means of achieving a legitimate aim for this post to be carried out by a female employee. We have assessed the duties of the role and determined that there are no alternative arrangements that would allow the work to be performed safely and effectively., Social Prescribing and Personalised Support

• Provide onetoone support, giving people time to tell their story and identify what matters to me.
• Coproduce simple personalised wellbeing plans based on strengths, priorities and goals.
• Help individuals address wider determinants of health such as housing, finances, social isolation, caring responsibilities and access to services.
• Support people to register with GP practices and navigate primary and community care.
• Make supported introductions to community groups, statutory services and VCSE partners.
• Maintain a small caseload, prioritising according to need and urgency.

Smoking Cessation Support

• Deliver one to one and group-based smoking cessation interventions on GRT sites.
• Offer behavioural counselling, motivational support and relapse prevention strategies.
• Educate individuals on risks of smoking, available treatments and coping strategies.
• Support access to nicotine replacement therapy or other cessation options under clinical guidance.
• Apply MECC principles during all interactions.

Outreach & Community Engagement

• Build strong, trusting relationships with GRT communities through regular site visits.
• Work collaboratively with local VCSE organisations, Traveller Education Services, community groups and statutory partners.
• Promote health awareness including vaccinations, screening, mental wellbeing and selfcare.
• Identify unmet needs and elevate gaps in provision to the Equity Manager & Clinical Lead.

Equity, Inclusion & Safeguarding

• Deliver care that is culturally sensitive, trauma informed and person centred.
• Work with interpreters, support workers and advocates where needed.
• Escalate safeguarding concerns in line with ABC and statutory procedures.
• Promote equality of access and proactively reach individuals who may be disengaged from services.
• Maintain accurate, confidential patient records (manual and electronic).
• Contribute to data collection and reporting for service monitoring and evaluation.
• Follow ABCs policies on infection control, confidentiality, data protection, and safeguarding.
• Attend supervision, mandatory training, and team meetings to maintain competence and reflective practice.
• Coordinate and plan GRT outreach clinics and manage time effectively.

Experience

• Experience working with vulnerable or inclusion health groups
• Experience in community, outreach or primary care settings
• Understanding of safeguarding adults and children, Experience delivering smoking cessation or MECC interventions
• Experience working with GRT communities
• Experience in social prescribing, community development or health improvement, NVQ Level 3 Health and Social Care, or equivalent qualifications or working towards, Training in motivational coaching and interviewing or equivalent experience, Excellent communication and interpersonal skills
• Ability to build trust with people from diverse backgrounds
• Strong organisational and time management skills
• Ability to work independently and as part of a multidisciplinary team
• Competent with digital systems and documentation
• Ability to identify and manage risk appropriately, Understanding of health inequalities affecting GRT communities
• Compassionate, flexible and trauma informed approach

• Valid UK driving licence and access to a vehicle

Our Equity Team delivers clinical outreach services to people who experience barriers to healthcare, including those facing homelessness, refugees and asylum seekers, Gypsy, Roma and Traveller communities, and other groups experiencing health inequalities.
