Band 5 Community Nurse – Kent Science Park, Sittingbourne
We are looking for a motivated and caring Registered Nurse to join our community nursing team in the Kent area.
The role works 7‑day service from 08:00 to 20:00 and focuses on supporting patients and their families throughout their rehabilitation journey.
Responsibilities
- Provide safe, high‑quality nursing care to patients and their families.
- Conduct thorough assessments and develop individual care plans.
- Coordinate with multi‑disciplinary teams to ensure continuity of care.
- Maintain accurate and timely clinical records.
- Communicate effectively with patients, families and colleagues.
- Implement best practice, contribute to service improvement and audit activities.
Qualifications & Experience
- Registered General Nurse with a current NHS license to practice.
- Band 5 experience or equivalent in a community setting preferred.
- Strong communication, organisational and IT skills.
- Proficient with Microsoft Office (Outlook, Word, Excel).
- Willingness to travel to and within the service area (minimum driving licence and access to a vehicle is essential).
