Job Overview
Wakefield North Primary Care Network (PCN) is seeking a Clinical Pharmacist to join a friendly and dynamic team of Pharmacists and Pharmacy Technicians.
A Lead Pharmacist (ACP) will support your training and development and act as a mentor when required.
The PCN operates six GP practices on the eastern side of the Wakefield District, serving a population of approximately 48,700.
We are looking for enthusiastic, forward‑thinking pharmacists to provide high‑quality care to patients, including the elderly, those with multiple comorbidities, and residents of residential or care homes.
Responsibilities
- Work within your clinical competencies to provide expertise in medicine management.
- Conduct face‑to‑face and telephone medication reviews.
- Manage long‑term conditions and support GP practice staff with acute prescription requests and medicine reconciliation on transfer of care.
- Address public health and social care needs of patients, including the elderly and care home residents.
- Provide clinical leadership on medicines optimisation and quality improvement.
- Manage aspects of the Quality and Outcomes Framework and participate in quality improvement projects to enhance patient outcomes.
- Contribute to operational efficiencies while delivering excellent service within general practices.
Qualifications
- Evidence of working autonomously or as part of a team.
- Medicines management and principles of Medicines Optimisation.
- Experience communicating with healthcare professionals and the general public.
- Ability to prioritise workload and use initiative to solve problems.
- Evidence of personal leadership to improve the quality of care.
- Knowledge of polypharmacy and deprescribing.
- Experience working in a primary care setting / PCN.
- Policy/procedure development for general practice repeat prescribing systems.
- Understanding of healthcare provision in GP practices, QOF, and PCN DES.
- Appreciation of the new NHS landscape and relationships between practices, PCNs, and commissioners.
- Proficiency with Systm One.
- Master's degree in pharmacy (MPharm).
- Registration with the General Pharmaceutical Council (GPh C).
- Evidence of continued professional development.
- Postgraduate Diploma in Clinical Pharmacy (or equivalent).
- Independent Prescriber relevance to primary care.
- Completion of the Primary Care Pharmacy Education (CPPE) pathway.
- Excellent time management, communication skills and data interpretation.
- Ability to manage a caseload of face‑to‑face clinical care.
- Adaptability and cooperation with multidisciplinary teams.
- Risk identification and appropriate escalation.
- Maintenance of confidentiality and professional judgment.
- Understanding of current healthcare policy, including the GP Contract and NHS Long‑Term Plan.
