As an integral part of the team the post holder will assess, plan, implement and evaluate the delivery of physiotherapy in the team. You will be part of the therapy resource providing an urgent care response, as well as supporting discharge from A&E. Providing assessment, support planning and interventions to meet the person's needs in the community ensuring the delivery of high-quality care. We are looking for an experienced, forward thinking Physiotherapists to join our Hospital at home therapy team here in Herefordshire. You will be working as part of an experienced, multi-disciplinary and multi-agency team alongside other Physiotherapists, Occupational Therapists, and Therapy Support Workers. This is a countywide service therefore holding a full driving license and access to a vehicle is essential.,
- Professional Qualification to degree level or equivalent in Physiotherapy/Occupational Therapy
- HCPC registered
- Post Graduate specialist qualification/significant post graduate experience, Broad and specialist knowledge of relevant patient conditions and therapy interventions
- Demonstration of knowledge of core and specialist Therapy skills
- Evidence of ongoing CPD in relevant area
- Experience of multi-disciplinary communication, assessments and record keeping
- Computer Skills including word processing and e-mail, NHS or equivalent experience in a variety of settings
- Experience of using/fitting adaptive equipment
- Community experience
