Specialist Physiotherapist Medicine in Penarth

We are excited to offer an opportunity for an experienced and motivated Band 6 Specialist Physiotherapist to join our innovative Liver Reconditioning Service in Cardiff and Vale UHB, supporting patients with advanced liver disease to optimise their health and independence from home. A team of dietitians, nurses and a physiotherapist work in collaboration with the inpatient hepatology team to identify, assess and treat patients who may benefit from a home-based nasogastric feeding and reconditioning pathway, providing outreach care.

Main duties of the job:

• Deliver specialist physiotherapy assessment and intervention to patients with complex liver disease in community settings.
• Design and implement individualised reconditioning programmes to improve strength, frailty and functional independence.
• Work closely with MDT colleagues to monitor clinical progress, outcomes and patient safety.
• Support patients to maximise independence in meaningful activities such as work, family roles and physical activity.
• Contribute to service development, evaluation, and the establishment of best practice models of care.

We are looking for a physiotherapist who:

• Has post-registration experiences at Band 5/6 level, ideally including complex medical, frailty, or gastroenterology/hepatology patients.
• Demonstrates strong skills in rehabilitation, functional assessment and patient-centred care.
• Is confident working autonomously in community settings.
• Has excellent communication, MDT and MI skills.
• Is motivated to contribute to an innovative and evolving service.
• Experience in chronic disease management is advantageous.

Person Specification:

Qualification:

• H.C.P.C. Registered Physiotherapist.

Experience:

• Significant post registration experience in a physiotherapy position.
• Experience as working as part of a multidisciplinary team.
• Experience of providing clinical supervision.
• Experience of working with older adults in a rehab setting.
• Experience of working with people with dementia.

Skills:

• Ability to demonstrate specialist skills within the field of rehabilitation.
• Ability to communicate well with healthcare professionals, people with communication difficulties and the public.
• Ability to motivate others.
• Adaptive team player.

Specialist Knowledge:

• Specialist physiotherapy clinical knowledge in the field of elderly rehabilitation.
• Evidence of relevant post graduate training courses.

Personal qualities:

• Ability to cope under pressure/resilience.
• Enthusiastic.
• Friendly and approachable.
• Demonstrates values and behaviours of UHB.

Desirable:

• Able to travel, in a timely manner to meet the requirements of the job, this may include cross site working or various locations, carrying sensitive information and bulky equipment (if applicable to role).
