The Social Prescriber will work as part of the multidisciplinary PCN team to support patients with non-medical needs that impact their health and wellbeing. Key Responsibilities Patient Support
- Receive referrals from GPs, practice staff, and the wider PCN MDT
- Conduct holistic assessments using a person-centred, strengths-based approach
- Co-produce personalised care plans with patients
- Support individuals to address issues such as:
- Social isolation and loneliness
- Mental health and wellbeing
- Financial, housing, or employment concerns
- Lifestyle changes (e.g. physical activity, weight management)
- Long-term condition support
- Provide follow-up support to help patients engage with services and build confidence
Community Engagement & Partnership Working
- Build and maintain strong relationships with local VCSE organisations, community groups, and statutory services
- Maintain an up-to-date directory of local services across the PCN area
- Promote community assets and support services within GP practices
- Act as a link between primary care and the community sector
Practice & PCN Working
- Work flexibly across five GP practices, attending MDT meetings as required
- Contribute to care coordination for complex patients
- Support PCN priorities such as:
- Health inequalities
- Long-term condition management
- Mental health and wellbeing
- Prevention and early intervention
Data, Monitoring & Reporting
- Accurately record patient contacts and outcomes using clinical systems (SystmOne)
- Collect outcome data aligned to ARRS and PCN reporting requirements
- Contribute to audits, service evaluation, and continuous improvement
- Ensure compliance with information governance and confidentiality standards
Professional Practice
- Work within the NHS Social Prescribing Model and PCN DES requirements
- Maintain professional boundaries and manage risk appropriately
- Engage in supervision, training, and continuous professional development
- Promote equality, diversity, and inclusion in all aspects of practice
- Experience working in health, social care, community, or voluntary sector roles
- Strong communication and interpersonal skills
- Ability to work independently and across multiple sites
- Experience supporting individuals with complex or multiple needs
- Understanding of health inequalities and social determinants of health
- Confident IT skills and accurate record keeping
Desirable
- Level 3 or above qualification in health, social care, or related field
- Experience working in primary care or GP practice settings

Nexus MK PCN offices are based in Linford Wood Milton Keynes and cover 5 practices which include Oakridge Park Medical Centre, Wolverton Health Centre, The Stonedean Practice, Sovereign Health Centre, Purbeck Medical Centre.
