About the Role

Community nurses make a difference every day by caring for patients in their homes.

As a Community Nurse you will assess the healthcare needs of patients and families, monitor the quality of care they receive, and be professionally accountable for the delivery of care.

The role involves providing acute nursing assessment and interventions in patients’ own homes, ensuring appropriate referrals to community services, and supporting patients to remain in their home environment.

You will work as part of a multidisciplinary team in Northamptonshire, aiming to keep hospital admissions and re‑admissions to a minimum and to ensure patients can return home as soon as possible.

Responsibilities

  • Assess healthcare needs of patients and families and use technology to update patient records en route.
  • Monitor and maintain quality of care delivered in patients’ homes.
  • Provide acute nursing assessment and interventions in the home setting.
  • Make appropriate referrals to community services upon completion of the care episode.
  • Co‑ordinate with multidisciplinary team to deliver quality outcomes and improve patient experience.
  • Keep hospital admissions and re‑admissions to a minimum and support timely return to home.

Qualifications & Experience

  • Registered Nurse with experience in adult community nursing.
  • Experience in acute medicine or emergency care.
  • Hands‑on skills in venepuncture, wound management, catheter care and palliative care.
  • Proficient use of System One Patient Electronic Record and general IT literacy.
  • Valid UK driving licence and willingness to use a car for patient visits.
  • Organised, adaptable, resourceful, confident and able to cope in diverse situations.
  • Strong leadership, management, communication, problem‑solving and good judgement skills.
  • Capability to work flexibly with minimal supervision and prioritise workload based on patient need.
