The role includes coordinating care services and client schedules, matching carers with clients, supporting the setup of new care packages, planning rotas, communicating with families and healthcare professionals, and participating in the on-call rota.
The role of a Care Coordinator
- Coordinate care services and client schedules
- Match carers with clients to meet individual needs
- Support the setup of new care packages
- Plan rotas to ensure continuity of care
- Communicate with families and healthcare professionals
- Record staff availability, absences, and emergency calls
- Participate in the on-call rota when required
- Willingness to cover the on-call phone (additional pay provided)

- £26,500 - £28,000 annual salary
- Refer-a-friend bonus of up to £1,000

About you:
- Strong organisational skills and attention to detail
- Ability to work confidently in a busy, fast-paced environment
- A minimum of 1 year experience, or minimum level 2 NVQ in Health and Social Care
- Full UK driving licence and access to your own vehicle
- Minimum of 12 months remaining on your right to work in the UK
