Role Overview
Assist registered nurses/practitioners to deliver care to patients and clients, working in partnership with other health care professionals, the patients’ families, and carers.
Deliver patient care as documented in the nursing care plan and carry out a wide range of duties under the direction of a registered nurse for the duration of the shift.
Responsibilities
- Provide direct patient care in accordance with the nursing care plan.
- Assist and support the district nursing team in caring for community residents who require nursing care.
- Travel between patients’ homes within towns and rural areas in a timely and efficient manner.
Qualifications and Requirements
- Enthusiastic, team‑oriented, and able to work independently.
- Aware of and committed to the modernisation of the service.
- Strong communication skills and professional demeanor.
- Ability to travel between patients’ homes across different settings.
- Desirable: ability to speak Welsh; English and/or Welsh speakers are equally welcome to apply.
