Cardiff and Vale University Health Board is seeking an experienced professional to lead a complex caseload within the Community Mental Health Team.
You will assess, plan, and coordinate care for individuals with dementia and mental health needs across multiple sites, supervising band 5 staff and students.
You will participate in MDT meetings, deliver group interventions, and work within CPA ECC guidelines, balancing clinical governance with service improvement while travelling between sites.
