Advanced Clinical Practitioner - Asylum Assessment Service in Liverpool

Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are seeking highly skilled and motivated ACPs/ ANPs to join our bank workforce with opportunities within our Community Services based within the Asylum Service.

• Assess, diagnose, plan, implement and evaluate treatment, interventions and care for people presenting with undifferentiated, undiagnosed, acute, minor, complex or vulnerability-related health needs.
• Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
• Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, as appropriate.
• Diagnose and manage both acute and chronic conditions, integrating drug and non-drug-based treatment methods into a management plan.
• Ensure any patient requiring referral for an identified health, social care or safeguarding need is referred or escalated through the appropriate pathway within the required timeframe., Full NMC Registration with no restrictions
• MSc in Advanced Clinical Practice is desirable; candidate who are working towards this qualification or who have relevant experience are encouraged to apply
• Non-Medical Prescriber V300
• Clinical Examination and Diagnostics Level 7
• Experience of working autonomously
• Experience of working in urgent care and/or primary care
