Overview

The postholder will manage a clinical caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. Responsibilities will reflect requirements of the NHS GP Contracts and may be subject to change as services adapt.

Clinical Responsibilities

  • Make self‑available to undertake a variety of duties including face‑to‑face, telephone and online consultations and queries, visiting patients at home, checking and signing repeat prescriptions, and dealing with queries, paperwork and correspondence promptly.
  • Make professional, autonomous decisions in relation to presenting problems which may be self‑referenced or referred from other healthcare workers within the organisation.
  • Assess the health‑care needs of patients with undifferentiated and undiagnosed problems.
  • Screen patients for disease risk factors and early signs of illness.
  • In consultation with patients and in line with current practice disease‑management protocols, develop care plans for health.
  • Provide counselling and health education.
  • Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
  • Record clear and contemporaneous consultation notes to agreed standards.
  • Collect data for audit purposes.
  • Compile and issue computer‑generated acute and repeat prescriptions, avoiding handwritten prescriptions where possible.
  • Prescribe in accordance with the practice prescribing formulary (or generically) whenever clinically appropriate.
  • Complete medical reports (including NHS and private services) as required.
  • Comply with all relevant practice policies and guidelines, e.g. prescribing, confidentiality, data protection, health and safety.
  • Commit to life‑long learning and audit to ensure evidence‑based best practice.
  • Contribute to evaluation, audit and clinical standard setting within the organisation.
  • Contribute to the development of computer‑based patient records and to the summarising of patient records and read‑coding patient data.
  • Attend training and events organised by the practice or other agencies where appropriate.

All information relating to patients, carers, colleagues, other healthcare workers or the business of the practice must be regarded as strictly confidential and may only be divulged to authorised persons in accordance with practice policies and procedures.

  • Implement and lead on the full range of promotion and management of health and safety and infection control as defined in the practice health & safety policy, manual and published procedures.
  • Use personal security systems within the workplace according to practice guidelines.
  • Awareness of national and contractual standards of infection control and cleanliness and the regulatory requirements.
  • Provide advice on the correct and safe management of specimens, including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements.
  • Use and ensure correct use of personal protective equipment (PPE) by clinicians, staff and patients.
  • Manage incidents of accidental exposure and maintain accurate records.
  • Conduct regular audits of infection control practices and intervene when weaknesses are identified.
  • Maintain clean and sterile working areas, ensuring adequate hand‑cleansing facilities, wipes and instrument management.
  • Escalate hazards and conduct remedial or corrective action where necessary.

  • Excellent written and verbal communication skills.

  • Good understanding of the national strategic context of the NHS.
  • Ability to make clinical decisions based on the latest evidence and local/national policies and guidelines.
  • Professional challenge of differing opinions as a patient advocate.
  • Basic IT skills in word processing, Excel and e‑mail.
  • Experience using IT‑based patient documentation systems.
  • Understanding of confidentiality, the Data Protection Act, clinical governance and audit.
  • Awareness of equality and valuing diversity principles.
  • Evidence of experience or competency in the form of a professional portfolio.
  • Ability to work on own initiative, knowing limitations of own competencies.
  • Car owner/driver with exclusive access to a vehicle.
  • GMC registered, not subject to suspension.
  • Qualified General Practitioner.
  • Currently on a CCG performers list.
  • Clear enhanced DBS check.
  • Evidence of CPD.
