Horsham Collaborative Primary Care Network is seeking an experienced Clinical Pharmacist to join the Horsham Collaborative PCN Team for a fixed term 12 month maternity cover.
You will work within a multidisciplinary team to provide a comprehensive medicines service, offer prescribing information and support safe, effective care in general practice.
You will lead medicines reviews, support quality improvement and audits, and deliver patient-facing clinics and telephone advice.
