Overview

Are you an enthusiastic, experienced Clinical Pharmacist looking to expand your clinical skills further?

An opportunity has arisen for an experienced Clinical Pharmacist to join the Horsham Collaborative PCN Team for a fixed term 12 month maternity cover.

You will work as part of a multidisciplinary team responsible for providing a comprehensive medicine service, providing appropriate medicine and prescribing information and advice.

The role is pivotal to improving the quality of care and operational efficiencies, requiring motivation and passion to deliver an excellent service within general practice.

Responsibilities

  • Provide expertise in clinical medicines management, structured medication reviews, long‑term condition management, medicines on transfer of care, systems for safer prescribing, repeat prescription authorisation and acute prescription requests.
  • Address the public health and social care needs of patients in the GP surgeries that make up the PCN.
  • Perform medication reviews for patients with polypharmacy, especially older people, care‑home residents and those with multiple co‑morbidities.
  • Lead quality improvement and clinical audit, managing aspects of the quality and outcomes framework.
  • Review optimisation for patients with single or multiple medical problems (e. g., respiratory, cardiovascular, diabetes) and recommend improvements to GPs and AHPs.
  • Collaborate with care‑home staff to improve safety of medicine ordering and administration.
  • Attend and refer patients to multidisciplinary case conferences.
  • Audit medicines associated with unplanned hospital admissions and readmissions, implementing changes to reduce prescribing in high‑risk groups.
  • Provide patient‑facing clinics and telephone advice for medication questions and concerns.
  • Respond to medicine‑related enquiries from GPs, staff, other healthcare teams and patients, suggesting and recommending solutions.
  • Follow up with patients to monitor the effect of changes and facilitate referrals.
  • Contribute pharmaceutical advice to the repeat prescribing policy within each practice and support the re‑authorisation process by reviewing patient requests and medicines reaching review dates.
  • Ensure patients have appropriate monitoring tests in place when required.
  • Advise on the development and implementation of new services that have medicinal components.
  • Develop specialist knowledge and provide specialist clinics in defined clinical areas.
  • Demonstrate sensitive communication styles with patients, families and the multidisciplinary team.
  • Participate in practice meetings, case conferences and CPA meetings as required.
  • Undertake clinical audits of prescribing, feedback results, and implement changes with the practice team.
  • Analyse, interpret and present medicines data to highlight issues and risks.
  • Monitor practice prescribing against the local health economies RAG list and recommend red / amber drug use.
  • Assist practices in maintaining a practice formulary hosted on each practice computer system.
  • Audit practice compliance against NICE technology assessment guidance.
  • Develop and deliver bulletins on important prescribing messages.
  • Implement changes resulting from MHRA alerts, product withdrawals and other local and national guidance.
  • Assist general practice teams to maintain CQC medication compliance.
  • Operate within personal competence and the GPh C professional code of conduct.
  • Maintain accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Prioritise, organise and manage own workload to maintain and promote quality.
  • Participate in quality governance systems with GP, practice managers and the practice.
  • Support public health campaigns and public health programmes.
  • Understand legal issues related to vulnerable adults, family violence and substance abuse, ensuring appropriate referrals.
  • Engage in personal and professional development with GP lead and attend mandatory training.
  • Adhere to organisational policies and procedures including confidentiality, safeguarding, lone working, information governance, health and safety.
  • Collaborate as an effective team member, support others, develop new ways of working and liaise with ICB pharmacists and other prescribers.
  • Identify high‑risk patients, manage risk, comply with health and safety policy, incident reporting and support infection control.

Qualifications and Experience

  • At least two years post registration working in primary care, community or hospital pharmacy.
  • Experience and awareness of the breadth of common acute and long‑term conditions likely seen in a general medical practice.
  • Ability to plan, manage, monitor, advise and review general pharmaceutical care programmes across core areas, including disease states and long‑term conditions.
  • Understanding and acting in line with GPh C professional standards for practice.
  • Good IT skills, including ability to record accurate clinical notes.
  • Ability to communicate effectively (written, verbal and non‑verbal) with patients, relatives, carers and the multidisciplinary team.
  • Experience of working under own direction in a patient‑focused environment.
  • Skill in analysing, comparing options and recognising priorities when problem‑solving.
  • Master’s degree in pharmacy (MPharm) or equivalent.
  • Registered with the General Pharmaceutical Council (GPh C).
  • Experience of System1 and knowledge of primary care networks and collaborative ways of working.
  • Preference for CPPE Clinical Pharmacist training pathway or similar; Independent prescriber qualifications or specialist post‑graduate diploma level experience.

Benefits & Salary

£25.50 to £28.50 an hour, depending on experience.

The Horsham Collaborative Primary Care Network is an NHS collaboration between four GP practices: Courtyard, The Village Southwater, Cowfold & Rudgwick.

The surgery teams work closely with each other, sharing expertise and resources to develop new services.
