Cardiff and Vale University Health Board is seeking a Rehabilitation Support worker to provide clinical support, assessment and treatment for patients in their own homes, delivering exercise, activity and treatment programmes prescribed by clinicians.
You will cover the East & North Hertfordshire areas, travel between locations using your own car, and contribute to the patient pathway from acute care to discharge while supporting discharge planning and MDT meetings.
