Senior Clinical Pharmacist

Join our Pharmacy department as a Senior Clinical Pharmacist, leading medicines optimisation and ensuring safe, effective medication management across acute and long‑term conditions.

Responsibilities

  • Provide expert clinical pharmacy input as part of the multidisciplinary team, including medicines reconciliation, clinical screening, and therapeutic drug monitoring.
  • Optimise medicines use through evidence‑based interventions and support prescribing decisions via clinical assessment, risk evaluation, and formulary guidance.
  • Lead the implementation of medicines optimisation strategies, ensuring prescribing is cost‑effective, safe, and aligned with national and local guidelines.
  • Contribute to the development and review of clinical guidelines, protocols, and policies, and monitor and report on medicines usage, compliance, and safety incidents.
  • Supervise, mentor, and support junior pharmacists, pre‑registration trainees, and pharmacy technicians, and contribute to training programmes for pharmacy staff and other clinical professionals.
  • Lead clinical audits, case reviews, and service evaluations within your specialty and represent pharmacy in multidisciplinary quality initiatives and clinical service reviews.
  • Ensure compliance with all regulatory and legal frameworks (GPhC, CQC, MHRA, controlled drugs legislation) and support the implementation of electronic prescribing systems where applicable.
  • Lead or support the development of new pharmacy services, clinical pathways, and patient‑centred care initiatives, contributing to business cases and strategic planning.
  • Maintain confidentiality of patient information in accordance with Health & Safety at Work Act and organisational policies.

Qualifications

  • GPhC‑registered Pharmacist.
  • Significant post‑registration experience in clinical pharmacy, preferably in a hospital setting.
  • Strong clinical knowledge and experience in medicines optimisation.
  • Excellent communication, leadership, and multidisciplinary collaboration skills.
  • Experience mentoring or supervising junior staff.
  • BOPA Passport and Clinical Pharmacy Diploma (mandatory).
  • Postgraduate qualification in Clinical Pharmacy (e.g. Diploma or MSc) – desirable.
  • Independent Prescriber qualification – desirable.
  • Experience in specialist areas such as oncology, surgery, critical care, or cardiology – desirable.
  • Experience in audit, policy development, and quality improvement projects – desirable.
  • Familiarity with EPMA systems and digital health tools – desirable.
