The purpose of the role is to support the General Practitioners by managing the workload associated with patient prescription requests, medication queries, medicines reconciliation, and to support the delivery of high‐quality, safe, and cost‐effective prescribing within the practice. The postholder will, in time, also be required to work within a larger pharmacy technician team providing both the support described above and dispensing services, as we train our existing dispensary staff.

  • Repeat Prescribing Management
    • Process repeat prescription requests in line with practice protocols.
    • Identify patients requiring medication reviews and arrange appropriate review with a clinician.
    • Resolve medication queries from patients, community pharmacies and care homes.
  • Medicines Optimisation
    • Undertake medication reconciliation following hospital discharge and outpatient appointments.
    • Support with medication reviews within scope of competence/protocols.
    • Identify and action prescribing safety alerts.
    • Monitor high‐risk medicines in accordance with local and national guidance.
    • Support implementation of local formularies and prescribing guidelines.
  • Patient Safety & Clinical Governance
    • Support systems for monitoring and recall of long‐term condition medications.
    • Assist with audits related to prescribing and medicines safety.
    • Contribute to significant event reviews involving medicines.
    • Ensure compliance with CQC standards relating to medicines management.
  • Patient Support
    • Provide advice to patients regarding medication use, synchronisation and adherence (within competency).
    • Support patients with medication queries and signpost appropriately.
    • Promote self‐care and understanding of prescribed treatments.
  • Care Homes & Vulnerable Patients
    • Support medication reviews and prescription processes for care home residents.
    • Liaise with care home staff and community pharmacies to resolve medication issues.
  • Collaborative Working
    • Work closely with GPs, nurses, clinical pharmacists, practice dispensary and administrative staff.
    • Liaise with community pharmacies and secondary care colleagues.
    • Participate in multidisciplinary team meetings where appropriate.
  • Information Management
    • Maintain accurate and contemporaneous patient records.
    • Use clinical systems (e.g. EMIS, SystmOne) effectively.
    • Ensure data protection and confidentiality in line with GDPR requirements.
  • Professional Development
    • Maintain registration with the General Pharmaceutical Council (GPhC).
    • Participate in appraisal and continuing professional development (CPD).
    • Work within competence and escalate appropriately.
  • Qualifications and Skills
    • Demonstrate an understanding of medicines optimisation principles.
    • Knowledge of high‐risk medicines monitoring.
    • Awareness of prescribing governance and patient safety.
    • Ability to communicate information effectively with people at all levels by telephone, email and face to face.
    • Demonstrate an understanding of confidentiality and data protection.

    • Completed the 15‐month PCPEP training programme.
    • Good clinical system IT knowledge.

    • Enhanced Disclosure Barring Service (DBS) check.
    • Occupational Health clearance.
    • Evidence of Professional Indemnity.
    • Evidence of Continuing Professional Development (CPD) commensurate with the role.
    • Registered Pharmacy Technician with GPhc.
    • Level 3 Diploma in Pharmaceutical Science (or equivalent).
    • GCS (or equivalent) English and Maths at Grade C (4) or above, Evidence of CPD.
    • Accredited Checking Technician qualification.
    • CPPE training on Medicines Reconciliation.
    • Experience of working in a community, hospital or primary care pharmacy (dispensary).
    • Understanding of repeat prescribing processes.
    • Experience of using pharmacy or clinical systems.
    • Experience of Primary Care Networks.
    • Experience of supporting care homes.
    • Experience of Quality outcomes Framework (QoF) and Care Quality Commission (CQC) Standards.
    • Experience of using clinical systems (EMIS/SystmOne).
