Surrey and Borders Partnership NHS Foundation Trust is seeking a Psychiatric Nurse to join the North and South GP Integrated Mental Health Services (IMHS) team. The role is part-time (3 days) with days discussed at interview, supporting PCN residents and participating in MDTs and community-based work.
You will assess mental health, develop holistic care plans with patients and carers, and collaborate with GPs, housing, social care and community connectors to address needs.
