Sussex Partnership NHS Foundation Trust is seeking a Community Nurse/OT to join the Dementia Care Home Support Team (DCHST) in West Sussex. The role focuses on improving outcomes for people with dementia, co-creating care and risk plans with service users and carers to promote independence and timely discharges.
You will work in partnership with GPs, crisis services and housing to support complex health presentations and provide specialist nursing/OT advice.
