  • Core Responsibilities
  • Deliver comprehensive primary care consultations through multiple modalities (face‑to‑face, video, telephone).
  • Manage acute and chronic conditions and conduct home visits when necessary.
  • Oversee chronic disease management and maintain high standards in patient care.
  • Provide domiciliary visits for housebound patients and conduct minor surgical procedures within appropriate clinical governance frameworks.
  • Establish and maintain effective referral pathways to secondary care services, ensuring seamless patient journeys and timely access to specialist interventions.
  • Lead and contribute to the practice’s continuing professional development program, ensuring all staff maintain the highest standards of clinical excellence.
  • Play an active role in clinical audit and quality improvement initiatives, driving innovation and ensuring the ongoing enhancement of care delivery.

Qualifications & Experience

  • Full GMC registration with a license to practice and inclusion on the GP Register.
  • Completion of GP specialty training (CCT or equivalent).
  • Extensive experience in NHS General Practice, demonstrating clinical excellence and a commitment to high‑quality patient care.
  • Experience supervising or mentoring junior doctors, medical students, or allied healthcare professionals.
  • Experience in primary care innovation or service improvement, including familiarity with EMIS, Systm One, or similar GP software systems.
  • Proven experience working in NHS General Practice and managing acute and chronic medical conditions across a diverse patient population.
  • Experience performing minor surgical procedures in a primary care setting.
  • Experience in designing or delivering educational sessions or CPD activities.
  • Experience working with vulnerable or marginalised patient groups.

  • Understanding and experience in chronic disease management programmes (e. g., diabetes, asthma, hypertension).
  • Ability to work effectively in a multidisciplinary team and manage clinical correspondence, referrals, and continuity of care.
