Severn Health PCN seeks a part-time Care Home Care Coordinator (18 hrs/week) to support care home residents through MDT coordination and information management.
The post is based at Prices Mill Surgery in Nailsworth with local travel to practices as needed.
You will maintain accurate records, collate data for MDTs, and liaise with GPs, pharmacists and admin teams to ensure timely, person-centred care.
