This is an 18 hours per week role. The Care Home Care Coordinator supports the provision of timely, safe and person‑centred care for residents in the care homes for older adults aligned to the Practice. The post holder acts as a central point of coordination for communication between the Practice, clinical pharmacists, GPs, nursing staff, administrative teams and care home personnel, ensuring that relevant clinical and organisational information is shared effectively. The role contributes to the delivery of the Enhanced Health in Care Homes model. This includes preparation for and follow‑up of multidisciplinary team meetings, management of routine annual and six‑monthly review schedules, management of new care home residents, maintenance of accurate documentation and assistance with improvements to information flow between teams.
We are looking for an organised, motivated and detail focused Care Home Care Coordinator to help us deliver high quality, well coordinated care for residents in our aligned care homes for older adults. This role will be based at Prices Mill Surgery in Nailsworth. You'll have a clear home base and team around you, while also working closely with colleagues across the wider Network. As part of the role, you will travel between practices when needed to support care home work, attend meetings, and maintain strong links with our member practices. Travel is local and planned, and you'll be part of a small, friendly team who work collaboratively across sites.
Responsibilities
- MDT Coordination and Care Home Support
- Attend weekly MDT meetings across the three linked care homes.
- Prepare MDT lists, collate observations and required information, and ensure updated data is obtained from care homes.
- Record attendance, document discussions and decisions, and track agreed follow‑up actions.
- Maintain accurate schedules and monitoring systems for annual and six‑monthly resident reviews.
- Share MDT summaries and relevant communications with appropriate colleagues.
- Information Management and Documentation
- Enter observations, clinical information and relevant numeric data into SystmOne in a timely and accurate manner.
- Monitor tracking tools and dashboards to ensure all relevant information is being captured.
- Ensure actions, tasks and updates are clearly documented and completed according to agreed processes.
- Ensure key documentation, including power of attorney information, vaccination consents, RESPECT forms and other care planning records, is actively gathered, reviewed and kept up to date where applicable.
- Ensure new care home residents are identified and reviewed in a timely manner, gathering all relevant clinical and administrative information required for MDT preparation and ongoing care coordination.
- Communication and Relationship Management
- Act as a liaison between care home staff, GPs, clinical pharmacists, district nursing teams and administrative colleagues.
- Develop and maintain effective working relationships with care home staff and key practice personnel.
- Support care home staff in understanding processes relating to power of attorney, vaccination consents, RESPECT forms and care planning requirements.
- Escalate identified issues or concerns in line with Practice procedures.
- Service Development and Quality Improvement
- Contribute to improvements in the organisation and coordination of care home workstreams.
- Share relevant information, updates and good practice with colleagues.
- Support audits and QOF/IIF reporting.
- Assist with the implementation of improved systems to streamline workflows and support EHCH delivery.
- General Duties and Expectations

- Work flexibly and adapt to changing service needs, including urgent requests.
- Support occasional vaccination or health‑promotion activities where required.
- Fulfil safeguarding responsibilities in accordance with local and national policy.
Qualifications and Skills
