Responsibilities
- Assess patients through home visits, plan and implement individualised care plans.
- Provide nursing care and safe, effective treatments in the community.
- Use a range of community nursing skills – including palliative care, wound management and catheter management.
- Work independently while collaborating with a multidisciplinary team (nurses, matrons, therapists, coordinators and local administrators).
- Record accurate patient data and monitor progress.
- Travel within the locality; full driving licence and access to personal transport required.
- Adapt to a 7‑day service with core hours 8:00‑18:00 and flexible shift arrangements.
Qualifications & Experience
- Registered Nurse (part 1) or equivalent.
- Current or pending NMC registration.
- Mentorship qualification (preferable, not essential).
- Evidence of community nursing experience.
- Strong communication, interpersonal and time‑management skills.
- Clinical reasoning ability and proficiency with specialist equipment.
- Good IT and keyboard skills.
- Ability to work autonomously and as a team player.
