West Suffolk NHS Foundation Trust is seeking an Assistant Practitioner to join the Suffolk Integrated Stroke Service within the Stroke Early Supported Discharge Team. You will work with therapists to deliver evidence-based rehabilitation to stroke survivors in their homes and communities.
You will manage a delegated caseload, supporting assessment, treatment and ongoing review of patients with non-complex needs, while contributing to six-month stroke reviews, service development and SSNAP data
