Cardiff and Vale University Health Board’s specialist stroke early supported discharge team is recruiting an Occupational Therapy Support Practitioner.
You will conduct home assessments, design rehabilitation plans and deliver interventions to maximise independence in patients’ own homes across Merthyr Cynon and Bridgend.
You will work with registered OTs, families and the multidisciplinary team, maintain records and participate in supervision and in‑service training.
