An opportunity has arisen for a Nurse Practitioner to join our friendly and supportive team. We are looking for a highly motivated and enthusiastic individual, ideally with experience of working within primary care, to work alongside and support our GPs and existing Nurse Practitioners. The successful candidate will have the opportunity to work across a varied and rewarding role, seeing patients with minor illness and acute presentations, supporting chronic disease management, and undertaking home visits as required., The successful candidate will play an integral role within our primary care team, providing high-quality, patient-centred care across a varied caseload. The role will include the assessment, diagnosis and management of patients presenting with minor illness and acute conditions, alongside supporting the management of patients with long-term conditions. The successful candidate will also undertake home visits where required., We are seeking a Nurse Practitioner who will provide high-quality, patient-centred care to patients presenting with a wide range of acute and ongoing healthcare needs. The role will include:
- Assessing, diagnosing and managing patients presenting with minor illness and acute conditions, working autonomously within their scope of practice.
- Providing appropriate clinical advice, treatment and safety-netting to patients, ensuring they receive the right care at the right time.
- Supporting patients with their ongoing medical and nursing needs, including contributing to the management and review of patients with long-term conditions.
- Requesting and interpreting appropriate investigations and making referrals to other healthcare professionals and services where clinically indicated.
- Undertaking home visits as required, providing assessment, diagnosis, treatment and ongoing management to patients who are unable to attend the practice.
- Working collaboratively with GPs, Nurse Practitioners, Practice Nurses, Healthcare Assistants and the wider multidisciplinary team to ensure continuity and high standards of patient care.
- Promoting health, wellbeing and self-management, whilst ensuring patients are fully involved in decisions about their care.
- Maintaining accurate and comprehensive clinical records and working in accordance with practice policies, professional standards and relevant clinical guidelines. Candidates should hold an Independent Prescribing qualification and be confident in assessing, diagnosing and managing patients autonomously within their scope of practice. We are looking for an individual who is committed to delivering high-quality care, works well as part of a multidisciplinary team, and is keen to contribute to the continued development of our Nurse Practitioner service., The successful candidate will be an excellent communicator, with a commitment to delivering high standards of patient care. You will be able to work effectively as part of a multidisciplinary team, demonstrate a flexible and positive approach, and have the ability to work calmly and confidently within a busy primary care environment.

- BSc (Hons) Nurse Practitioner Degree or equivalent qualification, or working towards
- Clinical Skills Level 7
- Paediatric Clinical Skills or undertaking studies towards
- Independent/Supplementary Nurse Prescriber, Excellent interpersonal skills
