Senior Care Co‑Ordinator – Oxford Health NHS Foundation Trust Forensic Service
Oxford Health NHS Foundation Trust Forensic Service is expanding and developing an integrated model of mental health care for men in prisons across the Thames Valley region.
In this role you will deliver high‑quality, safe, and effective mental health care and interventions to service users within the prison establishment, including management of self‑harm and complex needs.
Responsibilities
- Manage a caseload of prisoners, providing triage, assessment and evidence‑based interventions within the multidisciplinary team and care programme approach.
- Contribute to discharge planning and ensure continuity of care.
- Provide advice and support to prison primary health care staff and other prison personnel.
- Collaborate with internal and external partners, including health care providers, social care, and probation services.
- Deliver group interventions and maintain a person‑centred caseload management strategy.
Qualifications and Experience
- Experience working within the criminal justice system.
- Knowledge of forensic care practices.
- Experience providing specialist interventions for individuals with mental health challenges.
- Experience managing self‑harm and associated risks.
- Valid driving licence.
