Provide advanced clinical assessment, diagnosis and management of acute and long-term conditions using evidence-based practice and national guidance. Prescribe within scope of practice, order investigations, make referrals, and maintain competence across a broad range of clinical skills. Lead care for patients with complex needs, frailty, long-term conditions, those who are housebound and care home residents. Develop personalised and advance care plans, promote self-management, recognise deterioration, coordinate multidisciplinary care, support safe hospital discharge, and lead palliative and end-of-life care. Contribute to QOF, QIS and Enhanced Service targets. Work autonomously within professional, legal and organisational frameworks, demonstrating advanced clinical decision-making while maintaining accurate records, confidentiality, safeguarding responsibilities and compliance with professional codes. Communicate effectively with patients, carers, colleagues and partner agencies, ensuring appropriate signposting and escalation. Comply with health and safety, infection prevention, equality, diversity, confidentiality and all practice policies while engaging patients in care planning and service improvement. Manage workload efficiently and adapt to practice needs.

Clinical Care: Consult with patients, taking full history, assessing, diagnosing and managing acute and long-term conditions using evidence-based practice and NICE guidance. Prescribe and review medications within scope of practice and current guidelines. Initiate action if required ordering investigations or organising referrals. Maintain clinical competence in a wide range of skills required to meet patient needs. Proactively identify, monitor and manage patients with complex needs, frailty or long-term conditions. Lead on care for patients in residential/nursing homes and those who are housebound. Develop personalised care plans, including Advance Care Plans, and promote self-management. Recognise early signs of deterioration and intervene to prevent hospital admission. Provide timely clinical interventions and coordinate multidisciplinary and multi-agency support. Facilitate safe and effective hospital discharge in collaboration with discharge teams. Act as nurse lead for palliative and end-of-life care, ensuring alignment with the Gold Standards Framework. Support delivery of clinical targets including QOF, QIS and Enhanced Services.

Professional Practice: Work autonomously within professional boundaries, national protocols and local policies. Demonstrate advanced clinical decision-making and critical thinking. Ensure accurate, contemporaneous clinical documentation on SystmOne. Maintain patient confidentiality and uphold safeguarding responsibilities. Adhere to the NMC/HCPC Professional Code of Conduct.

Communication: Maintain effective communication with patients, carers, colleagues and external agencies. Provide clear signposting to appropriate services. Ensure accurate collection of activity data for audit and service evaluation. Seek support and advice from GPs and senior clinicians when required.

Education, Training & Development: Participate in the education and training of students and new staff. Support induction of new clinical and non-clinical team members. Maintain professional accreditation and engage in continuous professional development. Participate in annual appraisal and maintain a portfolio of professional development. Attend clinical supervision, mandatory training and practice meetings.

Clinical Governance & Quality: Contribute to clinical governance, risk management and quality improvement. Reflect on own and team performance, identifying opportunities for improvement. Alert colleagues to issues of quality, safety or risk. Work effectively with external agencies to meet patient needs. Manage own workload, time and resources efficiently.

Health & Safety: Comply with the Health & Safety at Work Act and practice policies. Use personal security systems and follow lone-working procedures. Identify and manage risks in daily activities. Maintain safe, tidy and infection-controlled clinical environments. Report hazards, incidents or concerns promptly.

Flexibility & Review: Significant post-registration clinical experience in primary care, community or acute settings. Experience managing complex, chronic and long-term conditions. Experience working autonomously and making advanced clinical decisions. Experience in care home or housebound patient management. Experience in palliative and end-of-life care. Masters level qualification in Advanced Clinical Practice (or working towards). Independent Prescribing Qualification. The role requires Independent Prescribing Qualification.

We are a friendly GMS training practice in the Beeston area of Leeds, operating from two sites with a patient population over 19,000. Strong Nursing Team including Nurse Practitioner, ACP (Paramedic) and Practice Matron. Training both Registrars and FY2s. SystmOne. Full range of Enhanced Services delivered. Member of the South and East GP Federation (SELGP). The practice is a proactive member of Beeston and Hunslet & Middleton PCNs. We are a democratic and supportive team. If you have any special interests to develop this would be supported and encouraged in the Practice. We have a diverse population of patients which makes work here stimulating and fulfilling.
