The predicted salary is between 26530 - 29114 £ per year.

Work Hours & Schedule

Operational from 8am to 6pm Monday to Friday and 9am to 5pm weekends. The post holder will be expected to contribute to the weekend rota.

Location

North and East Hertfordshire with travel across localities.

Responsibilities

  • Provide clinical support, assessment and treatment for patients in their own homes, delivering exercise, activity and treatment programmes prescribed by clinicians.
  • Support the patient pathway from the acute sector through to discharge and facilitate multidisciplinary clinical care.
  • Carry out non‑clinical activities such as clerical support to aid efficient team running.
  • Work under pressure, maintain flexible scheduling and good time‑management skills.
  • Order community equipment such as hoists and toileting equipment as needed.
  • Participate in team meetings and support discharge home to assess processes.

Qualifications & Requirements

  • NVQ Level III or equivalent knowledge and experience.
  • Experience working in a rehabilitation setting.
  • Experience working in NHS and/or community roles.
  • Full valid driving licence and access to a car for business purposes (unless disability exempt).
  • Ability to travel to various locations across East & North Hertfordshire.
  • Excellent time‑management and flexibility.

Fixed‑term maternity cover, ideally 9 months.
