Bydd gan ddysgwyr sydd angen cymorth Anghenion Dysgu Ychwanegol (ADY), Anableddau a/neu Anawsterau Dysgu a byddant yn cymryd y cam nesaf at eu taith tuag at fod yn oedolyn.

Mae'r cyfrifoldebau yn cynnwys:

- Cyfrannu at adolygiadau blynyddol sy’n canolbwyntio ar yr unigolyn, gyda chynllunio statudol fel Cynlluniau Datblygu Annibynnol, a chymryd rhan ynddynt.
- Gweithredu fel enghraifft, hyrwyddwr ac eiriolwr dros ddysgwyr gydag anghenion dysgu ychwanegol neu anableddau a/neu anawsterau dysgu.
- Gweithio gyda staff a gweithwyr proffesiynol eraill i hyrwyddo annibyniaeth dysgwyr.
- Sicrhau bod dysgwyr yn ddiogel ac yn cael eu cefnogi bob amser.
