20-24 hours per week working hybrid in person and remotely. We are looking for a part-time practice pharmacist to complement our successful and friendly village GP practice in Claygate.
Main duties of the job:
- 5 sessions per week.
- The ideal candidate would be an independent prescribing pharmacist with experience in prescribing for diabetes and other chronic diseases.
- They would be able to provide structured medication reviews and manage high-risk drug monitoring.
- They would be able to provide in-person clinics, but some virtual working for other elements of the role which would include responding to MHRA alerts, prescribing projects, and quality improvement work.
- They would be involved with working with Quality Outcomes Frameworks, locally commissioned services, and CQC frameworks.
- Above skills are not an absolute requirement: in-house training can be provided for the right candidate.

About us:
Capelfield Surgery has been established within the Claygate community for over 30 years and has a steady patient size of 8,000. We are a 3-partner practice with 4 part-time salaried GPs. We have been working with remote pharmacists, but now due to building reconfiguration, we would like to have a pharmacist team member providing in-person appointments.
Job responsibilities:
- Structured medication reviews: to be pre-booked either opportunistically by the clinical team if a recognised structured medication review is needed (and monitoring has already been arranged) or by reception from lists provided e.g. starting with patients with more than 10 medications who have not had a review in the last 6 months, high-risk groups such as patients on anticoagulants.
- Pharmacist can use admin time to pre-screen/identify the next group to be booked in.
- Further prescribing projects can be used to identify patients needing 30-minute or 20-minute reviews with the practice pharmacist.
- Ardens structured medication review template to be used to code records.
- Online consultations: queries about medications from patients via the website can be booked into these slots; the pharmacist can either respond via text or telephone.
- MHRA + CAS alerts: pharmacist to use admin time to respond to alerts. Record to be updated via Teamnet.
- Medicines monitoring: Support admin/phlebotomy team in ensuring medication monitoring is up to date. Reports can be viewed via Ardens searches on EMIS. Specific patients from these searches can be booked into SMR appointments where review of monitoring is needed.
- When possible, medication management on the day prescription requests can be viewed and any overdue monitoring/reviews identified and actioned.
- AccuRx/Florey responses: These could be allocated to online consultation slots on the day by the tradmin doctor according to capacity.
- DOCMAN: letters have been screened by Admin and either sent to the duty doctor to take action (where further management is needed) or if solely medication reconciliation needed sent to pharmacist.
- Med reconciliation template to be used to record changes made to medication. If the pharmacist is unclear of any reconciliation needed, they should liaise with the tradmin doctor on the day.
Person Specification:
Experience:
- Minimum of 2 years post-registration experience, experience in primary care, community, or hospital pharmacy.
- Essential: Experience of working in a multidisciplinary team (MDT).
- Essential: Experience in conducting medication reviews and managing patients with long-term conditions (e.g., diabetes, hypertension).
- Previous experience working within a GP practice or Primary Care Network (PCN).
- Desirable: Experience in audit, quality improvement, and clinical governance.
Qualifications:
- Essential: Mandatory registration with the General Pharmaceutical Council (GPhC).
- Essential: Clinical Pharmacy Diploma or equivalent experience/training.
- Desirable/Essential (depending on level): Independent Prescriber (IP) Qualification.
