  • Demonstrates the skills and judgments to implement the medical plan of care, nursing interventions and procedures as necessary for the care of the patient, with the guidelines of his/her licensure.
  • Completes monthly client progress notes and vital signs.
  • Administers and records medications as ordered by the clients’ physicians in accordance with the standards of practice.
  • Collaborates and communicates on a regular basis with the Program Director/RN and members of the Adult Day Health team.
  • Assists with feeding, toileting, ambulating, transporting and other tasks which may be required.
  • Assesses and identifies changes in clients’ condition and needs and reports to the Program Director/RN.
  • Assists as necessary in the delivery of other required program services.
  • Maintains confidentiality of client/employee information.
  • Maintains confidentiality of administrative, marketing and clinical business practices, policies, procedures, techniques and plans.
  • Communicates to the Program Director/RN all information necessary to promote and maintain the highest quality and continuity of client care
  • Maintains environmental safety for the client and the staff.
  • Accurately and safely utilizes all equipment and supplies appropriate and necessary for the care of the clients.
  • Consistently functions as a member of the program team in a positive manner.
  • Ensures adequate nutrition and assists in the supervision of meals and snacks to ensure compliance with each client’s prescribed diet.
  • All other duties as assigned.

  • All other duties as assigned.

Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence and service.

  • Current LPN license in MA
  • CPR certification required
  • One year of recent experience in an acute facility.
  • Experience with older adults preferred.

Purpose of Position

The Licensed Practical Nurse provides nursing services for clients in the program and works under the director of the Program Director/RN.

Schedule Details

Per Diem Hours, Rot. Days, Occ. Weekends and Holidays. Area: Upper Cape.
