Hours: Full time/Part Time Opportunities

Location: Heathfield/East Sussex

Hourly Rate: £13.50 p/h

Job Purpose

The role includes working within an allocated zone, responsible for evaluating and assessing new client packages, developing care plans, carrying out risk assessments, conducting reviews of existing packages, liaising with Commissioners and clients, evaluating care progress and recommending changes to the care plan if required. It also involves mentoring and leading a team of Care Support Workers, carrying out staff observations and supervisions, and being part of the on‑call rota on a rotational basis. A full driving licence and access to own transport are essential.

Role Responsibilities

  • Carry out assessments of all new packages.
  • Meet and greet potential private clients and/or their families.
  • Re‑assess existing packages when a client’s needs change, such as after discharge from hospital.
  • Carry out required risk assessments to ensure safe care delivery.
  • Update care plans, tasks, risk assessments, medication information and other care‑related documents within the client’s electronic care plan and care folder.
  • Undertake client reviews under the direction of the Care Coordinator and Deputy Manager.
  • Undertake staff observations, competency assessment and/or supervisions under the direction of the Manager.
  • Liaise with other health and social care professionals.
  • Undertake care work when required to meet the needs of the business.
  • Work hours to include a minimum of one evening and an alternate weekend.
  • Be part of the on‑call rota, ready to cover outstanding care visits at very short notice.
  • Attend Team Leader Meetings on a monthly basis.

  • Maintain your uniform and all company property issued to you to the standard compatible with Candlelight Care's expectations; a clean and smart appearance demonstrates professionalism.
  • Refrain from wearing jewellery, false nails or nail varnish while on duty, leading by example and promoting good infection control procedures.
  • Conduct yourself at all times in a cordial, respectful and courteous manner, even in difficult situations.
  • Maintain good working relationships with your office team, other care and support workers, clients and their families/representatives.

Essential Criteria

  • Available to work one evening and alternate weekends.
  • Adaptable and flexible to deal with varying needs and situations.
  • Be part of the on‑call rota.
  • Willingness to cover visits at very short notice when covering the On‑Call.
  • Previous experience within a care role.
  • Other caring roles (e.g. caring for family members).
  • Good working knowledge of care and support work.
  • Understanding and respecting the need for confidentiality.
  • Good working knowledge of Health.
