Candlelight Care is recruiting 2 new Team Leaders for the Glastonbury branch.

This is an exciting and varied role, which will include evaluating and assessing new client packages, developing care plans, carrying out risk assessments, reviewing existing packages, liaising with the Registered Manager and clients, evaluating care progress and recommending changes to the plan if required, supporting our team of Care & Support Workers, and carrying out staff observations and supervisions.

You will also be part of the on-call team on a rotational basis.

Role Responsibilities

  • Carry out assessments of all new packages.
  • Meet and greet potential private clients and/or their families.
  • Re-assess existing packages when a client is discharged from hospital with a change in needs.
  • Conduct required risk assessments to ensure safe care delivery.
  • Update care plans and medication documents in the client's electronic care plan and folder.
  • Undertake client reviews under the direction of the Registered Manager.
  • Undertake staff observations and supervisions under the direction of the Registered Manager.
  • Liaise with other health and social care professionals.
  • Undertake care work when required to meet business needs.
  • Work hours to include a minimum of two evenings and one alternate weekend.
  • Be part of the on-call rota, ready to cover any outstanding care visits at short notice.
  • Attend Team Leader Meetings on a regular basis.

Desirable Requirements

  • Availability to work alternate weekends.
  • Adaptable and flexible to deal with varying needs and situations.
  • Experience within a care role.
  • Good working knowledge of care and support work.
  • Understanding and respecting confidentiality.
  • Diploma level 2 in Health & Social Care or equivalent.
  • Good communication skills.
  • Ability to work without supervision.
  • Caring and empathetic approach.
  • Reliable and trustworthy.
  • Ability to manage your own time.
  • Good team player, supportive of colleagues.
  • Full clean driving licence and access to own transport.
